1. Acknowledgment of Risk
I understand and acknowledge that participation in volleyball training, practices, competitions, and related activities involves inherent risks, including but not limited to physical injury, illness, property damage, or even death. I voluntarily assume all such risks on behalf of my child.
2. Release of Liability
I, on behalf of myself and my minor child, hereby waive, release, and discharge VolleyUS Club, LLC, its directors, coaches, staff, volunteers, and facility providers from any and all claims, liabilities, or damages arising out of or related to participation in club activities, except in cases of gross negligence or willful misconduct.
3. Medical Authorization
I authorize VolleyUS Club, LLC and its representatives to obtain emergency medical treatment for my child in the event of injury or illness. I understand that I am responsible for any medical expenses incurred.
4. Transportation
I understand that VolleyUS Club, LLC is not responsible for transportation to or from practices, games, or events unless explicitly stated in writing.
5. Media Release
I grant permission for my child to be photographed or recorded during club activities. I understand that such media may be used for promotional, educational, or informational purposes without compensation.
6. Code of Conduct
I agree that my child will follow all club rules, demonstrate good sportsmanship, and respect coaches, officials, and other participants. Failure to do so may result in disciplinary action.
7. Parent/Guardian Certification
I certify that I am the parent or legal guardian of the registered athlete, have the legal authority to sign this agreement, and have read and fully understand its contents.
8. Electronic Signature Agreement
By checking the box below and submitting this form, I acknowledge that I have read and agree to this waiver and that my electronic agreement constitutes a legally binding signature.
9. USA Volleyball Acknowledgment
By registering, I also agree to all applicable policies, waivers, and requirements of USA Volleyball.
